There is a mod called "Special mobs" basically every hostile mob now have more than 7 variants, to invisible skeleton snipers to lightning Blazes

The "zombie awareness" mod could be cool for hardcore, mobs spawn way more, follow light sources and player sounds to track the player, and can sense when the player takes damage and walk to that exact location from hundreds of blocks away. Because of this each night starts out normal enough, but once even one mob finds and damages you it gets worse and worse from there because dozens of others all are notified when the player takes damage. Soon there are so many mobs the only thing you can do is pillar upwards / get in a box to hide.

I actually like the idea of forcing you to change up your food type, because in Minecraft, there’s no reason to have a bunch of farms if you can just farm one single food type, and there’s no reason to keep all these foods you get if, again, you can just eat one food type. I personally try to change up what I eat in Minecraft just so it doesn’t sit around in a chest collecting dust.

One interesting mod is Epic Seige. Unfortunately it’s still in 1.12 last I checked, and it might not be updated.
It basically “enhances” mob AI to an absurd degree. I’ll list off some features:
1. Passive mobs now act like wolves; attack one and they’re all hostile towards you. I still have a screenshot of some pigs just watching me from a distance. Horrifying... unless you see sheep gang up on wolves, which is funny.
2. Skeletons’ accuracy is much much higher, AND they’re always shooting at you, even if they can’t see you. You might be hidden in your house, but the skeletons will still let you know they know where you are 24/7
3. Zombies can place blocks and destroy blocks to get to your location. You think digging a hole will save you? Naw. They can smell you, and fight their way to you. Build up super high? Zombies will just build themselves up too.
4.You know how creepers just walk up to you, stop, and charge to go boom? Well not anymore! They now continue walking, while charging, and don’t stop. Your house is never safe, since creepers are now 100% more fanatic towards murdering you. Also, they can try to blow up a wall if they think breaking it well help other mobs reach you, so a house isn’t as effective.

There is this health mod out there that makes it so you can have certain parts of your body damaged, like a leg or your head, it also adds many healing items to heal those damaged limbs, it also makes it so if you say get shot in the head with a arrow you instantly die but not if you get hit in the leg, it's a very great mod that I think would fit in very good with the tough as nails mod and many more.

Hey just and idea for you, you have done mods that make Minecraft harder so now you should do mods that make the game easier... that way if someone wants to balance out and easy mod with a hard mod or vis versa they can... just an idea

Sadly Tough as Nails has been discontinued :(. I’m waiting for somebody else to update it as they have released the source code
