When Catholics says it's the body and blood of Christ, of course they know it is still a wafer and wine in its physical form. What they mean, is that it is blessed to spiritually become the body and blood of Christ and that's why they love, honour, worship and even consume it as well, following Jesus' self sacrificing instructions in the gospels.
