Here is an explanation for those who still don't get it:
The woman licking the knife is a foot doctor, and the knife she is holding is usually used to cut off foot diseases, fungus, etc. That is why the other surgeons are horrified because it's as if she forgot she just scraped off the disease from a patients foot.
