this is an absolutely tragic story caused by a single switch in the wrong position, and a crew that didn't notice it while doing the checklist. they believed they had a t.o config warning because the alarm for that and the pressurisation system was identical, if they had only remembered that these alarms occur at times that never overlap, all of these people would still be alive..
