I agree heavily with rule 4 but I do have an exception; the closer the players are to imminent danger or truly life or death scenarios, pay closer attention to the fine lines. As a player, a blind creature saw through my high roll on disguise, it didn't help that I knew that and the DM didn't. As a DM one of my players died because an orc could suddenly speak English and was able to quickly and in exact words give away the player's position to a guy who couldn't speak or understand orc'ish. I love when players take advantage of little rules to defeat stuff that they would almost certainly lose against in a straight up confrontation, I wonder if you feel the same.
