For me, it makes perfect sense what the difference is, without explanation.
It's pretty much in the name.
Polyrhythm means "many rhythms", Polymeter means "many meters (i.e. time signatures)".

Therefore, a 5:4 polyrhythm is playing 5 notes in the time of 4 notes, which, if you subdivide the pattern into groups of 20, a 5:4 rhythm will be played at 1, 5, 6, 9, 11, 13, 16, 17 and back to 1.
A 5:4 polymeter however, is like playing 5/4 over a 4/4 beat. For example, playing a repeated pattern of 5 crotchets.
