It's an external jugular vein. Titrating nitro drips quickly to 100 mcg is hard as modern nurses expect the patient's head to explode. Most important point of cardiogenic shock is DNR/DNI status. Ativan should be given to the nurses asked to run ketamine and IV epinephrine infusions. Intubated asthmatics run at 6ml/kg and 8 breathes a minute will need Ativan...and with paralysis and fentanyl. The most important cause precipitating DKA is insulin interruptus and finger stick abscentia.
