How do I get to the second page it says to scan a long receipt into parts I don't see this?

How much personal information must be provided? Name and address obviously. Anything else?

The problem with Checkout 51 one is that the quantity at item is limited for rebates and you don't know this until after you literally checked out with your items from the grocery store and by that time you can't fill out the rebate and you end up wasting money and become highly disappointed

I have Checkout 51 but at the bottom of my offer it says cannot be combined with any other offers. If I use a coupon with it, will they not accept it ?
