It's a meta-reference on the game's gameplay perspective. We all know the gameplay is based on turn-based combat, but Earthbound uses a first person perspective, that's why when in combat you see a full frontal image of the enemies. The cover is basically a first person view of Ness against a Starman, that's why you can see his reflection on the robot's visor.

Mother 2 was a massive success, because it was marketed as the sequel to the first Mother game of course, which was also a success. So naturally, Nintendo had to do something different in order to make it appeal to Americans so that it feels like a brand new franchise. The results were an amazing game that apparently stinks according to Nintendo of America.

Imagine the boxart for the original Mother game if it made it to the States. It would probably include a giant rampaging robot that resembled the Statue of Liberty.

Honestly I still can’t understand for the life of me how the box art didn’t contribute to the games sales. This is one of the most visually striking and eye catching game covers I’ve ever seen and should be one of the most instantly recognizable

Nowadays it may be a cult classic, but that doesn’t change the fact that it was a failure in Nintendo of America’s eyes. Which might be the main reason why Nintendo never translated Mother 3 in English. If Earthbound was a success, we probably would’ve had Mother 3 here in America back in 2006.
