I went to this exact same Otter "cafe" in Tokyo. I had to ask the staff the same thing about otters and water.
They showed me another room in the basement which was for staff only. This was a much larger room than the otter-visiting rooms which are tiny. This acted like a break room for the otters. They explained that they would switch out the otters, letting them take breaks in turns. This was to make sure that the otters that visited guests were energetic, while letting the tired otters rest. It's still not very humane conditions, but it's something.

This is an "asian small clawed otter", of the otter species, they swim the least and walk around on land a lot.

They still need some water which I'm sure the cafe has out back but you wouldn't want to be holding an otter when it's soaked 😅
