great video! (note: you don't use warfarin for DVT prophylaxis, it's for treatment once the patient has DVT or PE!) I love all of your videos.

Erm think its mainly because warfarin takes a while to take effect (2-3 days), as our bodies still have a supply of Vit-K factors even with inhibiting Factor 1, 2, 7, 9 synthesis. So docs usually chart heparin in between warfarin takes effect.

Also heparin is shorter acting, so able to reverse quicker, esp if patients are for surgery (e.g. with clexane).

Hey Doc. I am a 40 year old male firefighter who had open heart surgery to replace a leaky valve and to repair an aortic aneurysm. I am currently on Coumadin and Lisinopril (no other medications) b/c of my Mechanical Valve. I have the newest, latest, and greatest heart valve (ON-X Valve) which is in clinical trials for low/No dose Coumadin. I am currently trying to get back to work, however I am having a difficult time with this b/c there are many people who believe that you should be automatically disqualified as a firefighter b/c of this medication. I disagree and believe you can work and function as a firefighter if certain criteria are met to include: what type of dosage is the person on, the AGE of the person, what type of medications the person is on, and if the INR is controlled. I have been making this argument for about a year and have proven time and time again that many of the studies on Coumadin pertain to geriatric patients 65 and over. Your thoughts.

So, is the epoxide reductase inhibited competitively? It must be if vitamin K is given in treatment.

I have Q: why have we to start heparin with warfarin ? I know one reason because of long half life of warfarin but what else ??
