I sat my OSCE recently and I want to say a big thank you for your help. the night before my Osce I was so terrified and extremely nervous I couldn't read any thing else so I pulled up as many of your videos that I could find and I just re watch them all. the last one I watched before I fell asleep was this one and the next morning I just had a feeling to watch it again, which I did and my OSCE was based on a patient with end of life. Hi Violet, may i ask, i read a case note about CHF. The patient was having SOB and activity intolerance problems during planning however, during evaluation patient already deteriorated and needs EOL care. My question is, do i still use the SOB and Activity intolerance problem in identifying the main nursing addressed during patient stay in the ward?

Can you give some examples on how to outline nursing care provided or i should just copy what i have written in my planning?
