Hospital management games are spreading like, well, a superbug, with Two Point Hospital recently joined by Project Hospital on PC. Our resident management game obsessive Alice compares the two games to see whether Project Hospital or Two Point Hospital is the best there is.

If you are looking for games like Theme Hospital you've got a bit of choice. To be fair to them, both Project Hospital and Two Point Hospital have a very different flavour. One is about dealing with a tidal wave of comic diseases, while the other is a more serious take on the management genre. Which you’re going to enjoy more largely depends on what you want from a game. While this video isn’t a full Project Hospital review or a Two Point Hospital review (you can find one of those at the link below) it hopefully spells out the major differences and gives you an idea of what to expect.

If you want a deeper building experience and more realistic micromanagement, then maybe head to Project Hospital, which gives you more tools to deal with (and more problems to worry about). If you enjoy the puzzle of trying bring order out of the chaos of a swarm of sick people, then Two Point Hospital is the game for you - it’s almost an arcade management game, based on the speed of decision making and the ease of the building experience.

Either way there’s something to recommend about both and they will speak to very different players. I enjoyed both games but I got tired of Two Point faster. As a sucker for realism Project Hospital was exactly what I wanted, and as a hospital worker myself I appreciate the accuracy even more. The mods that add several new departments and dozens of new illnesses to the game are a great addition as well.

In my honest opinion, Two point Hospital whilst covering the theme of hospitals, is a far more gamified way of presenting it. The stars system, the yearly awards, the fact that you go from place to place and redo the exact same thing over and over on different sized maps. Every map has it's treatment of the day and after that it's figuring out how it works and then simply dealing with the hoard of patients.

Project hospital on the other hand is much, much more like a simulation. There's no tech locked behind needing to do new maps, and you can happily keep your hospital or even clinic, to a set size and optimize. It has real life diseases, and allows for far more player interactivity when it comes to treatment, so you might accidentally learn something. the way you organize the departments and have interactivity between them echos real world functionality, and thus, it ends up being a straight up hospital simulator, as opposed to 2 point hospitals hospital room manager style.

I will however agree with the above review, 2PH is a far more "Fun" game in that it might make you laugh and is far more light hearted. But for those who take pride in making things work, making it work efficiently or even just making a very pretty, real life hospital, then Project hospital is the stop for you.

I purchased both and have enjoy playing both. Basically 2PH is sleeker and easier to get into, but definitely a bit silly which is both a good and bad thing depending on what you want.

PH is a lot more serious, but I love the attention to detail. Sure it can seem busy and cluttered, but so do real hospitals. The doctor's offices, labs etc look just like real hospitals with all the equipment.

It's fantastic that PH gives you the option of placing prefab rooms but also let's you custom build rooms down to every piece of individual equipment. Likewise, it's great that you can personally get involved in running tests, diagnosing and treating patients if you want to.

I can never get enough of management games though. If they release another hospital management sim, I'd buy that too :)

I think it's down to personal preference. With 2PH I was bored after 10 hours but with PH I am 30 hours in and still going strong.

I am definitely going to get Project, because I love those kind of stressful games where you are squinting and sweating and don't know what's happening. I'm looking at you, Cook, Serve, Delicious!

It's basically a choice between something like a racing game and a racing simulator - they deal with superficially the same thing but approach it in wildly different ways. I personally prefer Two Point, but I'd almost call them different beasts entirely. I think Project Hospital could be a bit more rewarding experience, but really requires a lot more investment, I just don't have the time to dedicate towards it.

I actually like the "infographic" look of PH for the rooms and backgrounds, but hate it on the characters. Their walking animations are also horrible, it looks like they're just sliding their feet back and forth. I think I'd almost prefer it if the people looked like the prisoners in Prison Architect.

The one thing that stop me from playing Project Hospital is that I can't read the descriptions. It's so tiny...😪 😫

I have both. I find that I prefer Two Point. Project has its good points but gets bogged down fast and I can’t get past not being able to rotate the camera when I’m trying to place things just right.

4:36 For anyone trying to decide between these two in 2021, Two Point Hospital now has the ability to create templates of rooms that you can then use in any of the hospitals as well as having the copy/paste feature, now. You can build it, decorate it the way you prefer to, and even save multiple versions according to how much you want the room to cost or to have multiple orientations. I usually make a 3 star and 5 star version of each room.

Also Two Point has released a lot of really amazing DLC and even now they are STILL releasing new content for this game. And it's hilarious. 💗

A big difference between the two, and an important one for me, is that Two Point teaches you how to play the game while you're playing it, and Project will require you to spend a lot of time watching other people's tutorial videos instead of playing because it can't be bothered to teach you.

I remember playing hospital tycoon when i was a kid. I loved it. thought it was a bit cartoony with the fake illnesses but i adapted to it. just got project hospital a few days a ago and im addicted to it

2PH was on a try for free weekend on steam. I rushed through all the levels that weekend. It was funny and I like the graphics. But felt I was done with it after that weekend. Not sure where to take the game from that point as I basically did everything and saw all the funny illnesses.
PH is more challenging. I'm always afraid I now did something bad and my Hospital won't get out of debt. The camera can now rotate (although in 90 degree steps) and I can actually move rooms (as long as they aren't used, reassigning staff can be a pain and kicking out patients because you want to move the room can be awkward) which makes PH the winner for me. The graphics of PH are good when it comes to the equipment and structures. The people look a bit stiff to me, but it's a matter of taste and I got used to it.

As someone who has just started work as a junior doctor, I'm glad I came across this comparison video, whilst I was deciding what to play in my down-time... Seems like Project Hospital is the winner here for me! Thanks for the great video!
