Generally steering wheels are attached by a single bolt! They have to be torqued down Tina certain spec. The most likely scenario is that somebody's torque wrench was set incorrectly and did not get tightened enough. With driving on rough roads, it could shake a loose bolt right off, and then the only thing holding the wheel in place is gravity and the wires that connect to the airbag and steering wheel buttons. I've replaced the steering wheel on my other car so I've removed and replaced this bolt many times

I took delivery of a Model Y in August that had a steering wheel that was misaligned by 17 degrees from the factory. Service has to remove and reinstall the wheel.

I wonder if the issues are related.

If I have to suffer a steering wheel detachment, let it be in a Tesla (the only car in which I'm confident sans-steering-wheel)
