Oh, I forgot to add adding some medical scissors or a dedicated clothing cutter tool. Using a sharp ass knife can really suck at times.

I would, personally, upgrade those keyrings on your mace and whistle to D rings on the pack and heavy-duty keyrings on the item and carabiners between. Alot stronger and quicker to release. They can be very small, small or small-to-mid, whatever size you'd be good with.
That Leatherman IS AWESOME! I need one so bad! Lol
The SWAT-T tourniquet is good, but I'd add another already opened and ready to grab tourniquet like a SOFT-T or CAT, have it "stadged" ready and stuffed into the MOLLIE-like panels for quick grab'n use. I'd probably stick it on the whistle & mace side so it's not in the way and if one of your arms is down, you can hold your pack with your knees while you grab the ready tourniquet.
I'd ditch that medical tape and replace it with a cut down version of your pencile method; take a #2 pencil, wrap a good 1.5-2in thick bands around the pencil in 2 locations, then cut the pencil giving you two tight rolls. I would take my time wrapping the tape so it looks good, helps save space and makes sure the entire width is protected to maintain its stickiness.
I would definitely add more severe injury components to your 1st aid bit. Especially ina bug out disaster pack, last situation you wanna be in is more vulnerability than you may be in already. And you may be around family or friends when disaster hits, or just innocent others who've been severely hurt. Definitely look into "combat gauzes", not just powder. The powdered is good, but takes more equipment, more time, more skill and may just not work if too much blood is rushing. A combat gauze is the best of both worlds! And definitely get some compression packs as tourniquets are awesome, but 65% of the body doesn't for tourniquets to even be used.
And maybe even add another pack 1/2 the size pack of this one with a MOLLIE type attachment JUST for a good pack like an MRE, a few protein bars or other survival, long-lasting bars. Enough food to last you 30-40hrs rationing, or you and another for 24hrs rationing.
Oh one more thing, I'd look for a small heavy-duty tarp to replace that black bag. Great thinkin, but kinda the wrong material in that use. That material will tear easier, will hold in humidity and heat worse, if you have to setup shelter during the day due to weather or injury and it's summer do you really wanna rest under/inside sumpin that's gonna attract heat, how are you gonna strap it down? Does it have eye-hole grommets?...
Just my little thought process.

Other than that man, solid solid pack!! Got some great ideas off your deal here like that compass, fire pack setup, own that Streamlight too and LOVE IT, carrying a charger device (I only carry spare vehicle charger and wall charger), carrying more than a couple water purifier options and carrying that Leatherman tool pack too.

You need something for more serious injuries maybe a small compression bandage, quick clot sponge, tourniquet or at least some ABD pads sponges also having a washcloths in a ziplock bag to clean yourself is always a good idea because you can rewash the cloth and seal it up
