The mod is called "Remote Explosives" and is available on Xbox & PC. I'm not sure about Playstation because they have a weird modding policy.

Fadingsignal really does make some of the best Fallout 4 mods, I swear. My personal favorite is the West-Tek tactical optics, which adds a ton of stylish and functional NV goggles and visors.
