heres what i think and im going based of the books a little- bethany youngs picture has never been seen on pll but in the last episode melissa is seen pushing in bethany who coincidently has blonde hair and the same yellow top so i think bethany is really alisons twin. in the book it said that no one knew that alison had a sister but its later revealed that she was sent to a mental hospital and her name is probably changed because the family doesnt want anyone to know that someone in their family is insane (this also makes sense cause tanner mentioned that they couldnt find any family of bethanys). in a previous episode the girls were speculating why mrs.d didnt tell anyone who she saw hit alison with a rock and the girls assumed that she was trying to protect someone so it would make sense that mrs. d would try to protect her other daughter
