You're right, In my experience you can't typically go lower then CAT5E because lower spec cabling can cause issues when running PoE (Noise gets on the other pairs). I have heard of installations on cat3 but the phones were not PoE. Basically CAT5E or higher can't go any lower unless you really have to. There are some products out there that will condition cat3 and make it more up to spec for VoIP but I have never seen any real-world deployments of it.
