She Asked For Antibiotics & I said NO | Wednesday Checkup

Back at it with another patient story on this week's Wednesday checkup! A patient asked me for antibiotics in a situation where they were not indicated and I decided to use the situation as a way for us to broach the topic of overprescription of antibiotics.

Also, I changed up some of the details to protect the privacy of my patient so HiPAA is being adhered to.

This is SO important to talk about, especially the whole bit about antibiotics not working against viral illnesses. On the pharmacy side we are 100% always trying to limit the use of antibiotics as much as possible. At my community pharmacy we've even had to have conversations with local doctors about unnecessary prescribing of antibiotics. It's a huge issue that's really become bigger and bigger in recent years so thank you so much for bringing it up!

With antibiotic resistance going up, it's important to only give antibiotics when it actually is a bacterial infection rather than a viral one. I wish more doctors would say no.

Could you talk about migraines? I feel like it's rarely talked about and quite downplayed although it can have an effect on work and private life. I remember the first time I had a migraine with an aura I actually thought I had a stroke. Thank you!

I went to a doctor once for generic throat symptoms and fever. She suspected it was mono because the rapid strep test came up negative. She ordered a different strep test and prescribed me amoxicillin anyways.

I learned very quickly that taking a penicillin based drug during a mono flare-up causes a full body rash that lasts for several days after the medication is stopped.

I really wish that she had waited a day or two to get the results for the strep test she ordered to come back before prescribing antibiotics for me. Having a full body rash is not fun.

When we were covering antibiotics in pharmacology, our lecturer had said something about the treatment of viral upper respiratory tract infections along the lines of: “if we treat it, it’ll be gone in a week, but if we don’t, it’ll resolve itself in 7 days”.

My 3yr old and I live at the Urgent Care lol. Best advice ever given: Doc says, "If you're sick all over it's viral and wait it out. If you're sick in an isolated location such as ear, kidney, eye it's probably bacterial and need antibiotics." This advice has saved me a fortune and lots of time!
