Rose actually never appears in murder house while it's being filmed, but in later season apocalypse when different characters from a different season mesh together and go to the murder house And they find Constance now dead and part of the house. She decided to OD in murder House living room because three out of four of her children died inside the house. Adelaide the only one who lived to adulthood died outside not on the property so her spirit was not able to stay. It's never quite known what physical ailment was afflicted on Rose but apparently it was so bad that she hated seeing her own reflection so she gouged her own eyes out. The character we see in Apocalypse looks like a 3-year-old
