As a NICU nurse, I must say that a baby, whose heart condition is so severe and will definitely die without an immediate heart transplant, will not be active and looking pink like that baby Jack. He/she will be highly likely very pale/cyanotic from inadequate oxygen distribution, intubated to help with oxygen intake, and sedated to prevent pulling of any medical devices. -_-

That’s why these decisions do not go to the treating physicians and they are made by an unbiased organization and an algorithm based on the available information. The treating physician basically only decides if the patient is stable enough to undergo a transplant and discusses these things with the family. That’s why there’s a transplant list
