Do you always aim for head with flechette or is thorax okay as well

With the recent wipe/patch 0.12.11 Flechete and buckshot is pretty bad. They nerfed it to oblivion. Same with KS-23! So I recommend using AP-20s for your setup quest! GL friends!

Flechette only really works with Saiga drum mags, otherwise it's ass. Rather run AP-20 or 8.5
