Ques 1-: We have used here id, fname, email, phone, call1, budget, service, image, date, dob. Except fname all other fields are same as mentioned in form, can we use any dummy value to prevent SQL injection or is ther any pattern, as you used same except fname....??

Ques 2-: Why we have used "if ($stmt->affected_rows==1)" this if condition and why we cant use this syntax "move_uploaded_file($_FILES['image']['tmp_name'], 'upload/'.$image);", directly to upload image....??
