I was looking at shoes in a shop a few years ago, when an older gentleman approached me and struck up a conversation. He suggested going to a local leather supplier, buying scraps-by-the-pound, and tracing out your foot and/or the existing insoles and stacking anywhere from one to ever-how-many you might need until your shoes fit better. I've been doing this ever since, and it really helps dial in the fit of all of my shoes and boots!
