DC plane crash: What led to the tragedy?

The U.S. National Transportation Safety Board (NTSB) has launched an investigation into the deadly crash between American Eagle Flight 5342 – operated by PSA Airlines, which is a part of American Airlines – and a Black Hawk helicopter at Ronald Reagan Washington National Airport.

The plane, flying from Wichita, Kansas, was carrying 60 passengers and four crew members, while three soldiers were aboard the helicopter.

ATC is responsible for all air traffic in the area. The helicopter ceiling was suposed to be 200 ft the collision occoured at 450 feet, ATC should have noticed the descrpancy!!!
