Strategy: Die about 1000 times until you know his moves perfectly. Stay in front of him, but with a sort of constant "just to the right" momentum. Evade his attacks and wait for opening to hit his head, and maybe get some swings in on his arms. His legs/tail aren't very vulnerable. With the burial blade, when his head is at rest/default stance between moves - you can hit his head if you stand slightly to the right of it just in front of it from standing r1. This is because standing r1 has a high hitbox, but swings diagonally slightly to the left. The next attack in the combo is wide and low and will not hit.
It's important to know which attacks not to attempt to punish on. For example, the attack where he shoves both arms out - left then right - straight out. Sometimes this ends in a full collapse to the right, punish that. Sometimes it does not, and there's a really quick recovery on this move - so if you go in expecting to punish you will get wrecked.
In his third phase, arms ripped off, you really won't be seeing many opportunities to hit the head. You have to stand way too far away to evade his attacks so there really isn't a window to get in there even when his head IS exposed (which is rare). So I switch it up to focusing on the legs at this point. With his arms ripped off, his moveset in regards to standing around and behind his legs is reduced. Stomps and jump-aways only. You could probably use those stomp induced windows as a chance to hit the arms and body more than I did instead of purely focusing on the legs. You could also probably use that moment when there's a blood-splosion on the legs as a chance to go hit the head since it appears to hang low in response.
