PHM Webinar: Supporting Medi-Cal Members with LTSS Needs Experiencing Transitions of Care

On August 21, 2024, DHCS hosted a webinar providing an overview of transitions of care for Medi-Cal members with long-term services and supports needs. The webinar also featured an interactive discussion with an ECM and Community Supports Provider, Partners & Care Foundation, as well as a Medi-Cal Managed Care Plan Health Plan of San Mateo. This discussion focused on experiences standing up supports for individuals with long-term service needs, particularly as they are transitioning between levels of care.
