I was trouble shooting my sisters 08 Honda Element and figured I'd share a quick video. I almost took this car to a shop since I could not read it with my generic OBD2 reader.
Glad I found this info, hope this helps someone else.

For education purposes only.
Attempt at your own risk.

VERY IMPORTANT THAT YOU START WITH KEY OFF BEFORE JUMPING THE OBD2 CONNECTORS.

MAKE SURE YOU TAKE OFF THE JUMPER WIRE AFTER.

OBD2 pin to jump
Pin 9 and Pin 4

Each long flash of "D" = 10
Each short flash of "D" = 1
Long pause of "D"/ being OFF = its cycling to repeat.

Once you have the code count reference against the code below. In my cause its code 40 (due to 4 long flash and no short flash)
If you find more codes or you find an error please comment below for others to find.

05- transmission range switch
07- shift solenoid valve A
08- shift solenoid valve b
09- output shaft speed sensor

15- input shaft speed sensor
16- clutch pressure solenoid valve A

22- shift solenoid valve c
23- clutch pressure solenoid valve B
25- 2nd clutch fluid pressure switch
26- 3rd clutch fluid pressure switch
27- 4th clutch fluid pressure switch
28- ATF temperature sensor
29- clutch pressure solenoid valve C

40- torque converter clutch circuit
45- shift control system

61- shift solenoid valve E
62- transmission range switch

70- shift solenoid valve A stuck on
72- shift solenoid valve C stuck off
74- shift solenoid valve E stuck off
76- clutch pressure control solenoid valve A
77- clutch pressure solenoid valve B
78- clutch pressure solenoid valve c

Thank you so much. This video saved me time money and headaches. I had the D light flashing for a year when car engages the 4th clutch and I already replaced the transmission of my 03 accord v6. Your method didn't work at 1st trial because I used a paperclip that looked like metal but it wasn't conducting electricity. I struggled to find an old metal paperclip but once I found one, it worked fine and the display showed a long flash and 7 short flashes (27- 4th clutch fluid pressure switch). That matches exactly the gear at which the D light starts to flash when I'm driving. I went to Advanced auto parts, bought a replacement with a same color as the original OEM for around $60 and installed it in less than 10 mins and since then the D light never flashed again! Thanks!

I found this in the Honda Element 2007-2009 Service Manual
The functions of the fuel and emission control systems are managed by the engine control module (ECM) on vehicles with manual transmissions or the powertrain control module (PCM) on vehicles with automatic transmissions.
Self-diagnosis
The ECM/PCM detects a failure of a signal from a sensor or another control unit and stores a Temporary DTC or a DTC. Depending on the failure, a DTC is stored in either the first or the second drive cycle. When a DTC is stored, the ECM/ PCM turns on the malfunction indicator lamp (MIL) by sending a command to the gauge control module.
• One Drive Cycle Detection Method When an abnormality occurs in the signal from a sensor or another control unit, the ECM/PCM stores a DTC for the failure and turns on the MIL immediately.
• Two Drive Cycle Detection Method When an abnormality occurs in the signal from a sensor or another control unit in the first drive cycle, the ECM/PCM stores a Temporary DTC for the failure. The MIL does not come on at this time. If the failure continues in the second drive cycle, the ECM/PCM stores a DTC and turns on the MIL.

The first four flash codes are actually 50, 70 80, 90
40 - Torque Converter Clutch Hydraulic Circuit STUCK OFF
60 - Shift Solenoid Valve D

Other systems (ABS, VSA) have similar flash codes that can be read in the same way. Each flash code can have several P codes that narrow down specifically what may be wrong.

Thank you much. I was praying mine was just EC:26 and it was! 3rd clutch fluid pressure switch. Liked and Sub'd, thanks for actually posting out the codes as well.

I am getting code 21 .. on my 2000 honds civic CVT.. 2 long followed by on short flash of the D light. Cant find interpretation of this 21 code anywhere . Its missing on the list shared by the uploader aswell ... it jumps from 20 to 22 directly

My engine, vtm and tire pressure monitor are also flashing codes. Where can I go to get the description for these error codes?
