Thanks Andrew for your input in the case! we were just discussing how to choose the relevant FiO2 for your case.

Now most people would set the FiO2 at approximately 50%.

But here I make the case that for sick lungs that might progressively worsen, it may be better to start at a lower FiO2.

Prerequisites
1. Ensure sats are safe for patient eg greater than 94% with room for error
2. No other concerning features - difficult airway, shared airway, risk of airway loss

Why?
1. Identify lung deterioration as soon as it occurs
2. High FiO2 will mask deteriorating lungs over more deterioration
3. You have less time to solve the problem when you increase the FiO2 in failing lungs if you started with high FiO2.

It's also wise to not run extremely high FiO2 to prevent absorption atelectasis as well!

PaO2 around 70 (around 90%) is enough to keep tissue perfusion, there is no need to use high FiO2 beyond that, you could and you should improve MV configuration. Increasing FIO2 is an quick way to improve Sat, and give you time to think where should you adjust at your MV.

Why do you need reasons to do what is known today? Use the lowest fio2 to get a sat of 92-94%. That's it.
