It's because when you're playing missions you're the main character so you have more plot armor

One other cool hidden feature is that whilst roaming the mountains outside of missions you can encounter various wildlife, but THERE AREN'T COUGARS IN MISSIONS!!

One fun fact I don't see shared often is that in the opening mission with Franklin, the cop cars that chase you are pretty light and can be sent flying with Franklin's ability. I would always keep the cops on me because it's so fun flipping them over. It's weird how the cars seem to get heavier throughout the game

My guess is this was done for purposes related to dialogue.
In GTA4, it was kind of hilarious, but every time you'd crash or fly out of the windshield during spoken dialogue, it could cause the characters to have to repeat lines of dialogue over and over again after Niko recovered.

I think in some Trevor missions he does more damage, like it takes couple shots with a rifle to explode a car but it takes many outside of missions.

fun fact: lamar was going to replace franklin in the story, but lamar's voice actor was arrested and franklin stayed

I haven't played in a while but if you get on a motorbike and wait for a few seconds before driving, the character should put on a helmet. I always assumed that they would do the same with seatbelts in cars.

You can't fly out of your car in Online as well, which I wish wasn't the case, it would be so cool to have people ragdoll out of their cars.
