NTSB: One person dead after Southwest Airlines incident

Oh no...the lady who got partially sucked out died...how sad...prayers..

This engine did not have an uncontained failure. There was a failure of the engine inlet. the inlet is its own part attached to the engine. There are several reasons for it to fail. Though the #1 reason is due to the failure of the inlet anti- ice function that keeps the ice off the inlet. In my opinion the anti-ice duct came loose. Then the hot air that is ported off the engine. Causes a high pressure in the inlet barrel that when mixed with the heat causes the composite structure to lose strength. It then fails internally getting sucked into the engine, then the structure outside in the air flow has lost its internal structural support, and then departs the engine as seen in the pictures.
The unfortunate loss of life was most likely caused by the outboard engine fan cowl folding up and over the wing and striking the window as it too departed the aircraft.
I have been in the aircraft maintenance industry for twenty one years. And have seen this happen three times in my career.
My condolences to the family for their loss.

I am very sorry for the loss but even more grateful for one hell of a pilot and crew. Thank God for the engineering and assembly for holding together that which I see.

Can you all imagine the person sitting next to her? Who was it? I pray for that person and the others that witnessed this horrible event. I DO NOT BLAME the airline, however. From what I can gather, it was a freak incident.

The damage to the front of the engine shown in the video could never be engine failure damage. The containment shielding is further back where the jet's compression takes place. We're not being told the entire story. Something else cause the metal to enter the cabin if it came from the front/intake area of the engine.

Those passengers were Lucky only one passenger lost its life. Pilots did an awesome job to control the plane.
