This is the form I-864 Affidavit of Support. It should be filled-out or submitted by the US citizen sponsor or lawful Permanent Resident sponsor. I made a sample on how my husband filled out all the pages before I submitted it to USCIS together with the other documents for my Adjustment of Status application... This is based on our own experience and it might be a help for your reference as well.

Hi, If you have already submitted your Form I-864 without including income from a household member and now plan to add that income, how do you correct that? And also I plan on using my wife as the household member, but we filed jointly only on the 3rd recent(2021). Will that be a problem too? Thank you so much!!!!

I have one question, I’m the sponsor. And we got married so do I put my last name or his last name since we are married already? Like let’s say my name is kenny Julies (before I got married) and my husband name is Bob Tracy do I put Kenny Tracy(after Marriage) or Kenny Julies?
