I think that Sunny is trans MTF, but is in denial.

There is a literal gender change potion in headspace, which allows Omori (Sunny's dream version) to transform into a girl (but only in deep well).

Of Course, any other type of trans headcannon is also valid, i just think that the story potential of this potion are imense.
