For those who do not find that guy in the church sitting on the chair
and the witch in the ruins doesnt talk to you:

you have to port to the academy where you fought rennala, behind rennala is a large door, you need to pass it.
then you fight an invasion with her against jerren.

after that you have to port to primeevel sorcerrer azurs point, (pretty far up left on the map, you were there once, its where you got the comet azur skill) turn around and there is the set.

Before you can get the primal glintstone, first you kill radahn, then you talk to sellen at the waypoint cellar, then you talk to her at the witch bane ruins.

This was incredibly awesome. I couldn't get the seal until I returned to the sorcerer and then go back and talk to her body chained to the wall.
