If I understand correctly, the scene where adult Naomi wakes up from his coma and adult Ruri sits next to his bed is the real world. In the real world, Naomi was the one who got struck by lightning and became comatose, not Ruri. Adult Ruri and her scientists then created the ALLTALE and sent Naomi to the recorded world, or the first recorded world as we'll call it. Adult Ruri interfered so that she would get hit by lightning and Naomi would be okay. 

In the first recorded world, Naomi created the second recorded world, where the teenager versions of Naomi and Ruri live. Using teenager Naomi, adult Naomi turned teenager Ruri into a near-identical copy of the Ruri he knew, except she didn't get hit by lightning. With this, second recorded world Ruri was compatible with the body of first recorded world Ruri since they had the same memories. Syncing the mind with the body, as they called it. The key difference was that since this Ruri was saved from the lightning, she didn't believe she was in a coma. Therefore, by transferring second recorded world Ruri into first recorded world Ruri's mind, that believe would become reality, and she'd wake up from her coma.

However, this was all part of adult Ruri's plan. From the real world, she contacted teenager Naomi as the three-legged crow and gave him access to God's Hand once more. When second recorded world Naomi entered the first recorded world, she system realized there was an error and tried to erase it. They first went after Ruri, since she wasn't supposed to be alive in the first recorded world. When the Naomi's sent her back to the second recorded world, the system went after them. Adult Naomi sacrificed himself so that teenager Naomi could live with teenager Ruri in their original world, the second recorded world.

But adult Naomi didn't die. In the real world, Ruri tried to save Naomi using the same method Naomi had used trying to save the first recorded world version of Ruri. By making him believe Ruri was the one who got struck by lightning, Naomi didn't believe he was in a coma. His mind was able to sync with his body. Real world Ruri retrieved the consciousness of first recorded world Naomi and transferred it into his real body, allowing him to wake up from his coma in the real world.

I think the first crow (that stole kid naomi's book) was the moon ichyogo... It lead kid naomi to where adult naomi would be dropping into the world.

That crow is not the same crow that adult naomi had... His is a seperate crow alltogether.

The only time I think we see the ichiygo crow is when it stole kid naomi's book and lead him to adult naomi's "landing spot"... Then in the middle of the movie after kid naomi jumps into the void and ichygo crow then appears again and gives kid naomi back his powers.

What I think happened was that adult naomi entered the digital world and failed again and again and again trying to save Ichigyo. This endless determination made it impossible for him to get back to the real world. So when he finally saved ichiyogo he was still stuck and the only way he could leave the simulation was by giving up on trying to save ichiyogo.
