About 40 hours work per week, here no one counts for the hours, are just logistics on paper. When a resident, I worked only 5 official days- but 4 to 6 times a month we were “lucky” enough to be unpaid on-call (this meaning we would work for 34-36 in-hospital hours). And I would be naive to say after bring board certified this crazy hours stopped (even the professor in my clinic has more than 24h shifts). After finishing my residency I had 3 yrs equivalent holiday days to take (my residency =5yrs). In medicine, in my country almost nobody has all those 20 holiday days for good 😅

How does American patients treat doctors from abroad? Is there a lack of trust ? Right now i feel inclined to do residency in my home country and a Fellowship in usa.

These 80 hours include breaks ? How long are the breaks ?
And is it doable? To make my food, do laundry, work out and then do 80 hours in hospital?

If I understood it correctly, you get 55k yearly salary and you work as long as the Program need till 80h/week, that means there are no baseline like 8am to 5pm, there is just this 80h border that the Program cannot exceed. Im not sure if you will understand me. In my country there is 40h/week max, which means every hour after that must be paid separately, but in the US as I understand, they give you 55k and can make you work any hours long but till 80/week.

Dr. Malke, i was trying to apply for electives where step 1 status is not required. But all institutions like mount sinai, kansas a d harvard were saying that they will accept only LCME accredited schools and schools which are part of VSLO. IMGs will not be allowed for year 2022 to 2023. Is this realy still valid

Haha… you say something about annual exam. Bro, come in my clinic, weekly quizzes (sometimes very spontaneously random), JCs, presentations (like Sparta)- no one cares you just finished being on call and have 8 more hours to stay in the hospital. The feedback: is always public, and all the clinic will know how dumb I was in a test (not normalizing this). All my 5 training yrs (in Europe we have different training organization- all IM residencies have 5 yrs, except cards).
My board certification -17 mini exams in 3 days, both practical and theoretical. After every day I felt I survived another day in Squid game. 🎭
My point is competition mainly resides in yourself in most of the training programes in Europe
