Shaak Ti has FIVE different deaths...

Did you know that Shaak Ti has 5 different deaths, but there's one canon version of it!

Even though a majority of them aren't canon, Shaak Ti is still the only Jedi to have the most deaths.

She has yet another death in the cellphone version of _The Force Unleashed, _ in which Starkiller causes her to be consumed by carnivorous plants on Felucia.

I don't think the meditation is supposed to imply she is so deep in meditation she doesn't notice what's happening around her. I always interpreted it as her acknowledging the devastation surrounding her, and accepting that she will not survive the purge. I would say the scene almost functions as a willing execution. I think she knows the Jedi are being slaughtered, and she knows Darth Vader has entered the room. I think she is accepting her death and becoming one with the force (thought not in the way Yoda and Obi-Wan do of course).

In season 7 of the clone wars it is stated that she was sent to protect the chancellor but they had since lost contact with her, implying her capture. I still think it was grievous that killed her.

In the novelization of Revenge of the Sith, the Jedi Temple is placed on lockdown and orders are put out to not allow anyone to go in or out after Mace Windu and the other masters leave to face Palpatine/Sidious. When Anakin makes the decision to go to the Chancellor's office, he's met with Shaak Ti at the Temple doors, who reminds him that the Temple is sealed and that his place is to be at the Temple in the case it needs to be defended. Anakin storms past her, reminding her that he's the Chosen One, and therefore, "My place is there. I have to be there. That's the prophecy, isn't it? I have to be there. I'll do what I'm supposed to do." -Chapter 17
When Anakin returns later with the 501st legion, he asks Gate Master Jurokk where Shaak Ti is. He informs Anakin that she went into deep mediation in one of the chambers after sensing "something awful" in the Force. So therefore, I'll accept that Anakin is the one who killed Shaak Ti.

The Starkiller one is the best because it's the one that makes her look the most badass. She actually almost beat him, and he's the most over the top ridiculous Force wielder for a long time. He blew stormtroopers apart into dust like the Phoenix Force, he manhandled Star Destroyers with the Force. Shaak Ti nearly had him beat. This is the most badass death, it's the one I pick.

I don't know... The Clone Wars version looks more like Grievous was the one that killed her, it's shot for shot and the lightsaber that Grievous uses to kill her is blue.

I think all of Shaak Ti’s deaths are canon and she’s just the Mysterion of Star Wars, cursed to never stay dead and to have everyone forget her death every time she comes back.
