At first I was dismissing the theory that Kaato was the main villain but now it's space truckin time.

imagine if Kaato misses up and instead of curing Tsurugi, they fuse Tooru and Tsurugi together, and he becomes the main antagonist, Wonder of U gets a upgrade and adopts some of Paper Moon King’s ability, no idea on how that would happen and what his ability what be, but it sounds cool.

Ight but here's a serious theory; I don't think Caato's strategy is going to work
1) In a meta sense, they were definitely setting Go Beyond to be the way to cure the disease. It wouldn't make sense for the chapter that happened a few months ago that ended with "This is the way to overcome the family curse" to not bare relevance
2) Defeating the curse by sacrificing someone else was done before and it didn't work. Caato is just doing what she did to cure Jobin, which didn't fix how Tsurugi turned out
3) How would the rock disease affect Tooru?
4) Josuke being away at the hospital seems like he will return and help "fix things" per se. I don't think he would like Tsurugi being used like that

IDK It seems like to me Caato will be the final antagonist, but used in a way that is different to other "main" antagonists
