I have similar problem with my shoulder. Flat serves sometimes feel extremely painful, slice serves feel alright in most cases.

I don’t know if he faked it or not but I can say that I have pretty severe bicep tendinitis and when I serve sometimes I start out with a painful serve just like that but then push through the pain and hit good serves. sometimes it’s like a specific way you serve and it hurts extremely bad but other times you position your arm differently and it’s not so painful after.
