He also has a scar on that eye. That's why he uses it. From the messages, I think young Kaeya only wore the eyepatch while playing pirates with Diluc. He now wears it permanently to hide the scar. The teasing is just an addition here.

I think Kaeya is just fucking with us when it comes to the grandfather story lmao. My theory is that it’s a cultural thing. All of the characters we know from Khanriah wear some sort of covering on the right half of their face. In lore it stated that the last king of Khaenri’ah was known as the “One Eyed King”. And that king was missing his right eye. So I think it has something to do with that.

A theory also is he not only wears it to tease Diluc but also to help him as he knows Diluc would feel guilty seeing it and doesn't want him to be reminded/haunted by the fight everytime he sees Kaeyas eye so he covers it.

Actually At first he wore that eye patch to pretend like a pirate as his story quests tells us that his grandfather was one, and You see him wearing it when diluc’s father died after kof reached the area late. Because of the guilt Kaeya told diluc everything and diluc hurt his eye out of anger, that’s when he started wearing it permanently. It was also mentioned in the event lore that kaeya made diluc believe that he lost vision in that eye but diluc knew he didn’t. Maybe we’ll get to see his scar in future

Kaeya eyepatch story is just really suspicious if you think about it. One of the letters states that he alway wore the eyepatch since childhood and even kept wearing it in the manga. Even tho kaeya and diluc used to play pirate as a kid, they never mention that he only started wearing it because of that (even if he did, why did he keep wearing in the manga too?). not only that but why would kaeya keep wearing the eyepatch as a knight when he's not even blind? It can't be his scar because noelle saw it and he didn't seem to mind. If he didn't mind noelle seeing it, why would he mind if anyone seeing it either? Why would he purposely impaired his vision like that when it's really crucial for his job? Kaeya is just mad suspicious.

Edit: Actually, when reading some of noelle dialogue again, it said that she saw him buy multiple eyepatches and sometimes cleans/prepares his eyepatch. So do take that part lightly.

I feel like it's related to khaenri'ah Because he was wearing it even before diluc's father died. Plus all of the khaenri'ah seems to cover their right eye. For example: dainsleif, pierro(the dude from the fatui)

It also seems that his eye was blinded, but he reassured Diluc that he couldn't see from it before, to take the guilt off his shoulders. Covering the scar with the pirate eyepatch reminiscing memories of playing like they used to.
