This is still not all of the openings. It may be an alternate opening during season 1, but there is an opening that when the gunfire starts at the beginning, 5 bullet holes are shown with the Team's faces in them. For the DVD release they may not have used this opening since Amy was gone by the middle of season 2. But there is an opening with the 5 members of the teams faces design.

Missed a couple I'm afraid. The revised sequence for the second half of season two once Melinda Culea left / was forced out by George Peppard, plus season five had three versions I believe, although the one used was the best.
