How to create database table in visual studio 2010 , 2012 and 2013

Create database table in vs2010 and later version. Vs 2013 also create sql queries , you can use further. Step by step you can easy design table with some column

I did all that and still the table does not show up under the Tables folder.
I can see the saved table in my project folder "dbo.Table.sql" but it doesn't show in Server Explorer. ???
Also, when I click save is opens a full save as dialog with a *.sql default file type. I don't get the name field like you get. Why is this different?

Also how do you change the name of the table in 2013, the name field in properties is disabled. Can this get any more confusing? This stuff should be more intuitive.

The database I created shows on Server Explorer, but when I right click on "Tables", it only shows 'Refresh' and 'Properties', does not show 'Add new table' or 'New Query'. Any idea why?
