Megumi death ult should be him just summoning mahoraga since it's Canon to the series and the text can be something like
1st text "on the verge of death, he summoned it"

2nd text "MAHORAGA!"

To make it more balanced, u can probably do this after getting the wakening bar twice. Once u use it once ur awakening is gone. The second awakening bar is used only for death awakenings. And maybe after the awakening is over, they die. That would make sense for Itadoris ult anyway, his heart was ripped out.

Itadori (Vessel):
Dies, and then a Avatar from your friend list comes in, and foecefully feeds you multiple fingers. You get posessed by Sukuna, which rips off your heart. When this Short Awakening ends, you're dead.

Hakari (Restless Gambler):
Summons your Domain, while bleeding out. You only have one shot at getting Jackpot, or you'll die.

Gojo (Honored One):
"Oh... you're right... YOU'RE SO RIGHT!!"
Awakening obtained, but bleeding all over your body like Super Senior Gojo, you are "slowly" dying by the second.

Megumi (Ten Shadows):
On your knees, you realize you have no choice, but Summon Mahoraga. However, before starting the chant, you look down, and decided: "NO! SCREW IT! I'm gonna do it!!", before using your Barrierless Domain Expansion instead, a risky and gimmicky, but bold move. Even more so, with less than half of your health.

Mahito (Inspiration):
Inspired by your near death experience, you revive with your Awakening, but only at 20% HP.

Megumi: He tried summon mahoraga and decided not to, saying like "Ah nevermind! Here we go!". Since that happens when when he nearly died to the finger bearer.

Mahito: He stands up bleeding look at the sky, saying "What is this inspiration? Is this, death?". Referencing when he was inner monologue when he is getting jumped by yuji and nanami.

gojo (honored one):
*the player looking at their killer* first panel:"but on the verge of death..." second panel: "I finally grasped it, the true essence of cursed energy!" *the player appears to be bloody, has their ult, but has 50 percent health until they are killed or their ult runs out*
yuji/sukuna (vessel):
*gets up as sukuna* first panel:"I thought about what I can do right now..." *rips their heart out, eats a finger, then looks at their killer* second panel:"I'm taking this brat hostage" *the player gets their ult, gains 10 percent more ult, but once their ult runs out, they return to their normal avatar without the marks and die*
hakari (restless gambler):
*the player gets up and uses their domain, they get one chance, if they don't get a jackpot, they die, and if they do get a jackpot, the following will happen*: first panel: "*user name* truly has incredible luck!!" *they gain their ult, do hakari's dance, and everything happens as normal*
megumi (ten shadows): *the player gets on one knee and summons mahoraga* first panel:"hey you damn (cant say bastard)..." Second panel:I'll be dying first-" *mahoraga slaps them into the void, the player becomes mahoraga, and everything is as normal when you are playing mahoraga*
mahito (perfection): *the player gets the cosmetics for instant spirit of distorted killings and 50 percent more defense, but the armor can be broken with any black flash, for example, Yuji's black flash move. the player gains their ult, and everything happens per usual*
(extras for fun: Kashimo: *simply gets revived using lightning and gains their ult.* Geto/kenjaku:*stitches appear on the users forehead and they gain their ult.*
yuta: *user gets gojo's cosmetics, gets up, and unlike most character death awakenings, the player gains gojo's ult moves EXCEPT for infinite void, and gets infinite ult until they die.*

Mahito's death awakening could be that one time when Nanami and Yuji beat him so bad he unlock his domain, covers himself with his domain's hands before resuming the fight.

Megumi's death awakening can be this: when you ragdoll death, your body dispell revealing that was an actual shadow clone of Megumi's technique. The clone leave a shadow puddle where you come out again to fight.

Here a concept for megumi and Mahito. Warning may contain spoilers if you haven’t seen these but I highly doubt it

Megumi:when you get killed and use your ult your avatar would slowly get up from the ground and laugh as they put the hand sign for mahoraga as he would spear behind the player before doing why he would normally do and the sub title would be “I’ll be dying first, so give it your best shot

Mahito:so when you die you could have a 50/50 chance of succeeding this if you were killed normally but if you died by vessel’s black flash you would use the ult and when you use it a your avatar would get up before using a domain where it quickly expands then goes away before hands cover your avatar then they slowly reveal your avatar in the instant spirit body of distorted Killing and the sub titles would be “how about wishing me a happy birthday (Username)”

Megumi v2:(you would have to do a button combination or it would be based on chance)so it would start off with your character getting up before doing the pose that sukuna did when he entered megumi’s body before the marks appear on your avatars face and the subtitles would be “in a new vessel, A curse is reborn” and it could have a different move set but my fingers are getting tired and I think I did a bit to much thinking

Mahito's should be where he does his domain for the first time, which either breaks instantly putting his domain into cooldown or it uses up a second awakening bar like others said for balance.

I debated against that dude in JJS server, of about how this doesn't make sense at all, because to be honest, imagine you're 1v1ing, you win the fight but almost dies, then your opponent would simply appear full health with his ult on against you, who's just won a fight and has almost none hp.
