I've had to learn to bring in records. We got 3 different opinions on a cancer diagnosis, and I had to call, email, SNAIL MAIL, or fax all the imaging and test results to all of them. It was horrific. And that was only one patient, and me with adequate time to take care of it all!

I used to work in the legal profession, dealing with medical records.When my own Mom became gravely ill, and had to go to lots of docs and hospitals, I typed up a one-page overview of Mom's situation: each body system, treating specialist for that system with contact info, pertinent diagnoses and tests that had been done, with an overview of test results/diagnoses. Also added *my* contact info. I knew how time consuming it was to read medical records, and how unlikely it was that any doc new to her case would know anything about what was in those records. I got LOTS of grateful comments from nurses, docs & residents - they kept my cheat-sheet pronged at the top of her chart for quick reference.

ACE Inhibitors! (Category X) (As an Internist I had to know what G and P means AND what's safe in pregnancy as well as reminding everyone to check a pregnancy test on every female patient between teenagehood to 50!) I actually know of a surgeon who was fired for operating on a pregnant patient. Every MD should have the basic awareness of what to do for a pregnant patient!

As a PCP, my favorites are getting 5 notes about the colonoscopy procedure and no notes about patho. Same with colposcopy. And if there happens to be a path note somewhere- no instructions of when to repeat either procedure. I mean seriously people. 😣

You guys would get more records delivered to you by your patients if medical institutions wouldn't hold that info hostage. The person who has the least ability to access protected health information is the patient themselves. I used to work in records and the barriers to getting that access, including getting charged for it, were, more often than not, insurmountable. You as a total stranger, because of your profession, have far greater access that the patient.

I would argue that its very fortunate that the healthcare system isn't run by Elon Musk I don't see how he could make anything better.

Shouldn't the 'relevant' data thing be fairly easy to fix with a but of a concentrated effort?

Just include a search engine and have a representative body of healthcare practitioners assign tags for the data sets that allows a broader or more specified search for data pertaining to a particular specialty or idk, body area, time frame etc.
