The Kuchisake-Onna, also known as the "Slit-Mouthed Woman," is a terrifying Japanese urban legend that has been passed down for centuries. Legend has it that a beautiful woman, wearing a surgical mask, will approach you and ask, "Am I pretty?" If you answer yes, she will reveal her gruesome disfigurement: a grotesque, wide slit across her face, stretching from ear to ear.

The Kuchisake-Onna then asks the same question again, and if you answer no, she will mutilate you with a pair of scissors until you resemble her. But if you answer yes, she will proceed to cut your mouth in the same manner, making you look just like her.
