The reason why rod's titan is so big is because he is a royal blood and royal blood have the power to become one of the strongest titans but since rod never got a titan power he became an abnormal titan

The serum was from the colossal titan. His broken neck elongated his spine to this size. The rest of his prorportions are the same as the colossal. But he did not consume the shifter so he just got the mindless giant titan body.

My theory as to why hes crawling is cus when he consumed the serum he was crawling so he's crawling as a titan. Might not be true but it's a theory

His face was half off because he was dragging it on the ground.

the reason he was so big is because of he’s royal blood, has a broken spine, and has consumed the serum wromg but the reason his face is like that is because when he was moving it scraped his face off since we see his eyes when he’s a Titan before he reached the wall

i might be wrong but years ago when i read the manga, i remember there's a tiny details that says that serum is specifically a collosal serum
ofc the how big the victim can be and the size really depends on the individual
how they consume the serum has nearly 0 effect, for example in war Marleyan usually snuck the serum in wines, and in the wall rose invasion arc the villagers exposed with the serum from gas

Even though we don’t know how it here are 5 reasons on what that might have caused it:

- rod’s royal blood can alter the titan gratly

-the titan serum might have been "modded" like the armored serum

- how consumed the serum

- his broken neck could let the spinal fluid out and make it run through his muscles

- rod lack of space and aur in the chapel made it look like that as it could be "collosal serum" but the chapel just wasn’t tall enough

The reason why is because every fluid that contains Titans in it can be random at any sort of point you can have full control over it but it will be random you are correct

As you can see At the Rod Reiss Transformation and When the Titans go towards the Wall Maria you could see the Titan Eyes and Nose, Because Rod Reiss Drag his Face and Front Body on the Ground
