Hopefully, insurance companies made any formula for charging a ratio of bill to claim. there will be several stakeholders/especially insurance companies, who would have chalked out any specific returns to physicians and described co-payment amounts (What to pay or what not to pay)? why they would have to bill more than what we want. In this way, a doctor could be happy for the time being? But, I think this is the basic reason why the denial ratio of the insurance company is high.

Please consider from the insurance company's end why they would accept bluff bills unless certain rules are to be followed.
