Aatrox became beyond insane because of his desire to break free from his curse. He speaks evil so that heroes are determined to end him. For us, it's a win and lose for Aatrox but for Aatrox, it's a win-win situation because they get to kill him and he gets to break free from his curse. But due to the curse, he is unkillable. Aatrox's action aren't really bad, it's just a desperate action for freedom. But when Aatrox saw that no one can defeat him, he decided to kill all in order he can attain his end when there is no one left. Aatrox is more sad than Amumu for ngl.
