This grave and dialogue will only appear if you are in a New Game + cycle after having previously completed the "Yharnam Sunrise" ending. The pronouns change depending on if you're playing as a male or female character, indicating that this grave is actually yours.

I’m just gonna say this, because i know it hasn’t been recorded yet, but There was a specific and rare interaction with the doll that happened to me and it never happened again ever since. I approached the doll and she started talking weird… She sounded demonic and she was being possessed in a way. Maybe by the moon presence but right after, she went back to normal and it was like it never happened. I only know that it happened before i defeated the wet nurse but after defeating the One Reborn and going into the Nightmare of mensis. I’ll do my best to try to record it but i know that’s the only interaction not yet documented or talked about at all.
