D#1100 Append inputfield and remove after modal confirmation with verified checkbox

append multiple input field with help of select tag and remove the appended
input field after clicking on cross button with modal pop up are you sure
you want to delete appende input field. if yes appended field will be
deleted or else it will not be removed.
